Stimuli: SMR accuracy in 22/23 to cohorts outlined in Network DES was only 54% and no incentive in 23/24
How: Created searches to match ePACT2 polypharmacy comparators design in SystmOne, matched eligibility to each SMR Network DES specification cohort in SystmOne and validated cohort size to ePACT2 data. Assessed cohort demography and IMD decile and published information for pharmacy teams by PCN and practice in PowerBI to enable intelligent comparison of population need and past performance.
Published monthly performance tracking to enable ICB to support PCN in achieving submitted ambitions for SMR achievement chosen by PCNs matched to population need for their communities.,
Created data linkage from SystmOne reporting to data warehouse collated daily to enable SQL analysis of SMR effectiveness.
Evaluation of unique medicines prescribed in 90 days before SMR in BND Chapters 1-4, 6-10 and in the last 90 days over a five-month period (Oct 23 – Feb 24).Results: Change in SMR accuracy from 54% to 78% in 23/24.
Number of people provided SMR from Network DES cohorts rose from 14,500 to 17,170 in a year without incentivisation.
Unique medicines count dropped in people prescribed 10 or more medicines before and SMR by 8.5% saving an annualised cost of £303,101.
