We used a whole system approach, working with a range of healthcare professionals as well as organisations, to improve the asthma care in our Primary Care Network (PCN), whose population is affected by the high deprivation across the Borough.
We have set-up a pharmacist-led PCN clinic to review children and young people aged 5-18 years old with known risk factors to asthma attacks (SABA over-reliance, recurrent exacerbations, A&E attendances) supported by social prescribing link workers, GP care co-ordinators and secondary care.
Pilot data revealed a significant reduction in the risk factors for an asthma attack. Our project is ongoing and currently extended to target a wider at-risk cohort.
