We welcomed Professor Emma Baker, Professor/Consultant in Clinical Pharmacology and Consultant Physician St George's Hospital, London, specialising in Clinical Pharmacology and Therapeutics.
She leads a polypharmacy specialist service in South West London, with the aim of enabling better medication reviews across the whole integrated care system.
Emma talked to us about how she and her team approached the challenges of structured medication reviews, including taking a risk:benefit approach to deprescribing, having complex conversations with patients and when to ask for help.
