"What good looks like" for people in care homes to have fast access to COVID-19 symptom relief (2020)

Project Summary

In April 2020 during the COVID-19 pandemic, the northwest of England reported rapid deterioration of some elderly care home residents. Of concern, was the unpredictable and sudden nature of the symptoms: over hours, they had become agitated, delirious and breathless while coughing uncontrollably. People required fast access to medicines to relieve these symptoms.

Medicines' procurement and supply in England is tightly managed, and the pandemic was likely to put an unexpected pressure on this system. It was therefore critical to balance the preservation of the medicines supply chain whilst ensuring fast access to symptom control for those in need: in effect, it was critical that equitable, safe access to medicines for those living in care homes was designed, and designed-in, rather than added on.

The Medicines and Pharmacy Cell Northwest, led by the Regional Chief Pharmacist, created a Task and Finish group to explore “what good looks like” for patients to receive medicines at bedside within 1 hour of diagnosis, and to influence its design and implementation across the Northwest ICSs. 

Outputs included: 

  1. An illustration of the scale of the problem – the volume of NICE NG163 medicines potentially required in each CCG was modelled, and this was shared with each “place” in the northwest
  2. Clarity on “What Good Looks Like” for each health and social care sector to work as a joined-up system so people in the community could have fast, safe and legal access to medicines at the end of life with Covid19 symptoms. This 4-page document was shared across northwest region primary care and out of hospital cells and teams, as well as nationally.
  3. Covid19 hubs in primary care/communities were designed with fast, safe and legal access to medicines for people at the end of life with Covid19 symptoms
  4. Care home and prescriber support packs were published – 15 sets of guidance and 6 sets of proformas/templates
  5. Weekly northwest regional position of stock levels of end of life medicines sent to all prescribers, Covid19 hubs, and pharmacy professionals across the northwest of England
