Since the letter from NHS E on the 1st of May, Luton CCG Medicines Optimisation team have been providing comprehensive support to all our Care homes. We participate in all weekly MDTs and Check-ins, providing clinical and medicines management support, as part of the COVID-19 response.
This submission details the work we undertook that provided the basis for this, and describes the processes and the outcomes. Despite the Challenges due to the COVID-19 pandemic, the work is on-going, and we continue to work towards implementing the framework for Enhanced Health in Care Homes.
