The impact of the Pharmacy Team within Community MDTs (2022)

Project Summary

The Community Health and Wellbeing MDTs (CHWT) began as a pilot in one of our clusters at Cwm Taf Morgannwg University Health Board (CTM UHB) in 2017, and due to the success, in 2019, funding was obtained to roll the service out to the other clusters. In terms of pharmacy input, the original pilot had only an advanced pharmacist, but for roll-out, the inclusion of a senior pharmacy technician was recognised as a valuable addition to the team, allowing a prudent approach to medication-related interventions. The CHWT Pharmacy team are able to conduct structured medication reviews and have numerous interventions that not only promote cost-effective prescribing, but ensure patient safety is maintained.

The pharmacy team are valued members of the CHWT, having significant impact on patient care and showcasing the skillset of Pharmacy Professionals. The team have integrated well and been available for vulnerable patients within the community. Aligning to the principles of 'Pharmacy, Delivering a Healthier Wales (PDaHW) and the Royal Pharmaceutical Society's publication on MDT working, the team work tirelessly and provide a service of significant value.

The addition of the senior Pharmacy Technician has maximised the impact of the pharmacy team even further.
