Safety Review in GP practice of DOAC patients by a pharmacy technician (2019)

Project Summary

The aim of the audit was to ensure all patients were receiving the correct monitoring of their DOAC in primary care and on the correct doses to reduce the risk of bleeding. Three GP practices in Hywel Dda University Health Board were used to undertake the audit.

From the 3 GP practices, data was collected for 362 patients prescribed a NOAC.

  • 105 patients were identified as needing an up to date bodyweight (to ensure more accurate estimation of renal function).
  • 59 patients were identified as needing an up to date blood test for the monitoring of their DOAC.
  • 12 patients were on a dose higher than recommended for the renal function leading to an increased bleeding risk. Patients were referred to the GP and dose was reduced accordingly
  • 3 patients were on a dose lower than recommended leading to increased thromboembolic risk. Patients were referred to the GP and dose increased.
  • Call and recall for annual blood monitoring and face-to-face reviews were scheduled for each patient to ensure safe procedures in the future.
