Project Summary

Virtual MDT review collaboration between community care services and Luton CCG Diabetes Specialist Pharmacists to review the need for insulin administration by visiting district nurses to vulnerable, severely frail patients.

The MDT approach combined the efforts of specialists across primary care to address over-intensive management of people with Type 2 Diabetes with insulin, particularly those with advanced frailty, or with existing evidence of hypoglycaemia. This group of patients were also at a higher risk of exposure to Sars-CoV-2 virus by visiting healthcare professionals.

Predicted savings around £265,000 per year, not including avoidance of possible hospital admissions for hypoglycaemic episodes, risk of falls and extended periods of rehabilitative care in homes and higher risk of mortality by exposure to the Sars-CoV-2 virus.
