Pharmacist-Led Inclisiran Implementation: A Sustainable Model for Cardiovascular Risk Reduction (2025)

Project summary

To improve lipid management in line with NHS England’s medicines optimisation priorities, a collaborative process was developed between a PCN pharmacist and practice nurse to initiate and embed inclisiran prescribing in primary care. The model aimed to optimise cardiovascular risk reduction while reducing GP workload and streamlining service delivery.

Initially, the protocol was co-developed by the pharmacist and practice nurse. The pharmacist reviewed and initiated treatment, completed PSD documentation, and the nurse administered the injection and managed patient recalls. This shared-care model ensured immediate feasibility and safe governance.

To enhance sustainability, the pharmacist subsequently completed subcutaneous injection training. By the end of this month, the pharmacist will have completed the full patient journey — from assessment and prescribing through to administration — enabling a fully pharmacist-led service. This further reduces burden on nurses and ensures that GPs are not involved in any step, avoiding increased workload.

The project included the development of:

  • A bespoke SystmOne template for lipid review and inclisiran prescribing.
  • A practice protocol aligned with NICE guidance.
  • PSD documentation for nurse or pharmacist administration.
  • An administration record to support audit and data entry.

Seven patients have already received inclisiran through this pathway, which improves access, efficiency, and cardiovascular outcomes. The model is scalable, sustainable, and fully embedded within routine practice IT systems.
