Project Summary

Optimisation of medical therapy for heart failure can help prevent multiple hospitalisations and slow disease progression. GP practice based pharmacists were upskilled to provide HF clinics within patients GP practice. They were involved in optimising evidence based medicines with support from the local HF team.

12 month project. Patients were identified in 2 ways, PUSH: Patients identified by the Bridgend HF team (Dr Wong, HF nurses, pharmacists)
PULL: Patient identified by HF audits undertaken by practices to identify stable patient, who were NOT medically optimised based on current guidance/evidence.

Patient invited to Practice pharmacist HF clinics (in their own respective surgeries)
Ongoing review undertaken at practice level, inc bloods
Discussion at MDT if needed (weekly)
Referral for post-OMT echo once complete. See attached for full evaluation information
