Medicines Management Transformation Programme (MMTP) delivering sustainable medicines value at scale

Project summary

Medicines-related cost improvement programmes (CIPs) are essential for NHS sustainability but are often delivered in fragmented, short-term ways that undermine clinical engagement, transparency and long‑term value. At London North West University Healthcare NHS Trust (LNWUH), this challenge was addressed through the creation of a Trust‑wide Medicines Management Transformation Programme (MMTP).

MMTP brought together all medicines-related efficiency, quality improvement and sustainability activity into a single, clinically led programme with clear governance, shared ownership and robust benefit tracking. A dedicated Lead Pharmacist for Transformation role was created to provide strategic oversight, continuity and clinical credibility, linking pharmacy, finance, clinical teams and executive leadership.

The programme introduced pooled medicines targets, standardised benefit methodologies and a monthly programme board chaired by the Chief Pharmacist, reporting into executive transformation governance. Real‑time project tracking and analytics improved transparency, confidence in reported savings and proactive pipeline development. (see supporting evidence)

Through MMTP, a wide range of initiatives were delivered including medicines waste reduction, biologic pathway optimisation, reductions in unnecessary intravenous medicines, improved use of patients’ own medicines and data‑driven prescribing review. These interventions improved patient experience, reduced unwarranted variation and embedded medicines optimisation into routine practice rather than isolated schemes.

MMTP delivered £2.0m cash‑releasing savings in its first year and has delivered a total of £5.3 million savings since its inception. Beyond financial impact, the programme strengthened organisational culture, shifting conversations from siloed target delivery to shared value, sustainability and patient‑centred care. Designed for replication, the MMTP model has been shared across North West London and elements of it nationally.
