We have developed a timely and reliable process to inform GP practices of any specialist medication provided by Homecare Services. This aims to improve safety by reducing the risk that GP prescribe medicines without being aware of specialist medicines supplied via Home Care.
This has been achieved by leveraging existing IT resources in new and innovative ways, and creating a collaborative working process between different teams across Primary and Secondary care.
