Improvement of outcomes for cardiovascular diseases in Norfolk and Waveney has been achieved by active collaboration between partners in the healthcare system, public health, academic health science network, British Heart Foundation, Healthwatch, health education England, and community pharmacies. We are increasing the detection, diagnosis and treatment of atrial fibrillation (AF) and hypertension in or population. Both hypertension and AF, if left untreated can result in strokes and heart attacks.
Our approach constitutes of -system collaboration, design of projects based on deep knowledge of our population, selection of areas to focus, use of complex intelligence and information systems, clear communication of current and relevant information to clinicians.
Our data shows reduction in the number of strokes by 25 and heart attacks by 94 in the past year. The number of people being treated for hypertension and AF have also increased.
