Improving Medication Ordering Processes in Care Homes (2019)

Rotherham CCG

Project Summary

Our project was designed to help streamline the monthly ordering process in care homes to ensure all medication is ordered in a timely manner to reduce missing doses, relieving pressure on GP practices and promoting the carrying forward of stock to reduce unnecessary waste.

It has helped to synchronise new patients and new medications to ensure that items are in stock when needed. It has helped to improve the communication and understanding between care home and aligned GP. It has also helped to increase safety of residents by ensuring that all records, both MAR sheets and GP records are accurate and up to date, ensuring residents only receive current medication.

We have saved over £12,000 across 7 care homes in a 6month period but have also found it has proved successful in saving time for both care home and practice staff, allowing them to focus more on patient care.
