Project Summary

This project incorporated a whole suite of work to improve the safety of DOAC prescribing and monitoring. A non-valvular AF DOAC position statement was agreed across the whole STP footprint, incorporating the views of all healthcare professionals across the area. Prescribing and monitoring of DOACs was reviewed and updated as necessary and patients were switched to the preferred DOAC where appropriate. The medicines management team worked with GP practices to improve monitoring processes in order that the long-term safety of DOAC prescribing will be improved.

As a result of the project many patients across Derby and Derbyshire CCG have had the monitoring of their DOAC updated and many have also had their dose altered accordingly. Monitoring systems in GP practices have been improved as has the safety of DOAC prescribing. At the same time positioning edoxaban as our preferred DOAC for NVAF has funded the project and also produced significant savings for the CCG.
