Improvements in patient care are generally delivered by employing additional staff and running additional clinics to reach more patients. We have used the data and care plans developed automatically by Eclipse Live (Advice and Guidance) to re-design the way in which patients with Diabetes are managed within primary care. Practices are able to use the system to find patients who are missing one, two or three of the targets and follow up these patients in a systematic way based on prioritisation parameters set within the system, and using a range of healthcare professionals.
Practices have improved the delivery of the 8 care processes, and their individual practice % of patients achieving 3TT. The CCG is slowing showing an improvement in overall achievement.
The practices where this new approach has been embraced have been thrilled with this new way of working and keen to expand into other long term conditions.
